First of all, we would like to thank you for accompanying us to the Rustavi Mental Health Center today, your support is greatly appreciated.

As discussed, could you please share the below mentioned documents with us (sending documents in Georgian language is fine, I will translate them into English)?

- Contract models and financing instruments (“global budget” and “vouchers”);

- Sample Monthly Report (including the data on patients and their diagnosis) that is sent by Mental Health Centers to the Ministry of Health; preferably, one of the monthly reports produced by Rustavi Mental Health Center;

- Sample Funding Request Form that is necessary for getting financial aid from the MoH.
