Dear Sopo,
Some time ago you asked me about the planning 2020-2021.
Planning is still in its early stage. We worked on it a lot, as we have gone through several internal processes, and now we know, that we cannot plan any substantial increase in funding. Even expecting the funding that was available for 2018-2019 is a bit optimistic.
In the past months we translated initial proposals received by you into a comprehensive table summarizing the expectations of the member state(s). the table follows generic principles set globally for all member states.
This means that the areas proposed by you, have been grouped according to the predefined technical areas, that follow 13th General Programme of Work and enables uniformed reporting across the organization.
Due to the limited funding and ongoing activities and expectations for future, we may need to agree to delete some of the proposed activities, or to merge them under more generic titles, that would give us more flexibility during the implementation phase.
For now I have marked some of them in red. I think, they should not be included in the forthcoming BCA.
I have also added the area of environment and health, that has been just recently proposed by communication between Amiran and Piroska Ostlin.
Please, have a look.
This is still draft version that we may discuss towards end of February and final form of the BCA 2020-2021 will most probably be agreed between you and the new WHO representative later in spring.
Some comments:
1.1.1.: I think that he following two can be deleted, as electronic medical records are already being introduced; additionally this could partly be covered by the PHC strategy
COGEO assist introduction of electronic medical records;
COGEO assist introduction of modern methods of feedback on health risks and preventive interventions;
1.1.2.;
this among others includes different diseases, including vaccine-preventable infections (just for easier understanding, so you know, why so many different things are in one box).
New registries – I would also add modernize registries, as by this we cover both (new and existing registries).
1.1.4.: we may wish to narrow the scope of work of the following:
COGEO setting up monitoring mechanisms for national strategies and action plans to increase accountability and transparency;
1.1.5.: This is very bug new area; perhaps it is more realistic to plan it for as late as 2022-2023
COGEO analysis of Human resources for Health in Georgia to quantify HRH needs incl. policy options for filling the gaps
1.3.1.: I think that prescribing medicines belongs more to define basic health package; and the control over what is prescribed could be somehow addressed by strategic purchasing/selective contracting; consequently my suggestion would be to delete the below area:
COGEO assist polypharmacy decreasing mechanisms, incl. introduction of e-prescription tool
1.3.3.: by my opinion GMP inspectors have been trained; they do not need major support for performing their work and consequently we could perhaps delete the below – in red; on the other hand I see a need for further regulation of pharmaceutical market as well as pricing of medicines, which both goes much beyond GMP (text in green)
COGEO provide capacity building and consequently assist implementation of the GMP strategy; COGEO assist develop pricing policy and advance in regulation of pharmaceutical market
1.3.4.: not sure if we shall keep the below; perhaps it could be deleted:
COGEO foster Collaboration with stakeholders for processing intellectual property and leveraging on TRIPS flexibilities
3.1.2.: as per recent correspondence we may add environment and health, as follows below in green:
COGEO Assessment of WASH in health care facilities; Elaboration of WASH Strategy in Georgia; improve capacity for Health Impact Assessment
3.3.1.: as we already have 3.2.1 – multisectoral strategy for health promotion, as well as environment and health (3.1.2.) we may wish to delete the below one – on healthy municipalities, and work with them within the framework of multisectoral strategies and environment as indicated.
COGEO assist developing the strategy of healthy municipalities
By my opinion deleting the areas in red would reduce the number of technical areas and streamline collaboration, in particular having in mind limited funding and limited staff, as well as limited absorbing capacity within the ministry and possible other national institutions. This would contribute to better focus of the BCA and more strategic approach.
Please, have a look, and as indicated we may discuss towards end of February.
