3. 4. ICMJE form: Please also download the ICMJE form from the cloud folder, then save the form and re-open it from your saved file using Adobe. Fill in your name, complete Sections 1-5, and click the orange “Generate Disclosures” button in Section 6. Then save your completed form and upload it as a PDF to the above platform. Note: if you open the form with an internet browser, the form will not open.
