This email brings to your attention the following matters requiring your action by no later than Friday 7 February 2020.
- 2020 CONFLICT OF INTEREST – ANNUAL DECLARATIONS FORM: To be filled by ALL Board members/Alternate Board members/Committee delegates/Independent Experts on Committees
Please read the attached Conflict of Interest Policy for Governance Bodies and procedures documents before completing the form and which are included as a link to the form in English and French. Board Members and Alternate Board Members must complete a declaration of interest form annually. This extends to include Committee delegates and Independent Experts on our Committees and advisory bodies.
This is a continuing obligation and the Secretariat is required to be notified immediately of any changes to your declaration, should any changes occur in the course of calendar year 2020.
