Thank you for agreeing to welcome and make opening remarks at our upcoming Reporting on Immunization program.  Can you please use the link below to register by February 14 so we can make appropriate logistical arrangements.

In addition, we would like to include information about you in the meeting brochure and ask that you please email me the following by February 17:

·         short 200 word maximum biography (not a CV)

·         photograph/headshot of yourself

·         your credentials (name, degrees, title, organization) as you would like it to appear in the meeting brochure

 

In recognition of your contribution to the program, we would be pleased to offer an honorarium of $1,000 USD payable to yourself or your employing organization. Payment will be made by wire transfer after the conclusion of the event.

 

Sabin will provide transportation, lodging and an expense allowance for meals. Our team will contact you regarding your arrangements once you have completed the registration form.
