Dear Tamar Barkalaia,

You have been nominated by a colleague to complete a 360-degree feedback appraisal on them as part of the UNICEF LeadOn 360 Assessment. The 360-degree feedback process is being run by an external company, Envisia Learning.

Your individual responses to the UNICEF LeadOn 360 will be computer scored and combined with results from others who have also completed the questionnaire. The feedback from all raters will be summarized in a comprehensive summary report. Individual responses cannot be identified, unless you are the individual's Current Manager.

The UNICEF LeadOn 360 Questionnaire should take no more than 15 minutes to complete. At the end of the questionnaire, you will have an opportunity to provide responses to open-ended questions on strengths and opportunities for development. These comments can be helpful in interpreting the quantitative data.
